Your First Therapy Visit
Welcome to MIDSTATE Therapy Services. We are glad that you have chosen our facility to be the provider of your therapy services. It is the goal of our staff to provide you with high quality rehabilitation but also to make the experience one that is less stressful and much more enjoyable. Below you will find information to help you be prepared for your first therapy visit. If you have any questions, please feel free to contact our office and one of our staff will be more than willing to provide you with the assistance you need.
Patient Checklist for the First Therapy Visit
Bring Insurance Cards and Identification
Be sure to bring a drivers license or other form of identification in addtion to a copy of your current health insurance card(s). Also, if you have the original copy of your physican referral or order be sure to bring it with you.
Have all Paperwork Completed Prior to Your Appointment
All patients need to complete the forms included in the New Patient Packet. Click on the link to download the forms. Please complete all forms and bring them with you to your appointment.
It is important to arrive at least 15 minutes early prior to your scheduled apppointment on your first visit. This allows our staff adequate time to check you in, discuss insurance information, and answer any questions. There are occasions when you will need to complete paperwork prior to your appointment so arriving early is encouraged.
Make Sure You are Dressed for Therapy
It is important that the therapist is able to perform adequate examination of the involved area. Please be sure to dress so that the therapist can examine the area, especially on areas such as the shoulder, hip, or knee. If you would like to bring a change of clothes with you, then you will have the opportunity to change clothes at the clinic. Also, the therapist can provide you with a gown or shorts for the examination. Make sure your clothing is comfortable and does not restrict movement as you may be performing exercises. Also, make sure you wear comfortable shoes for walking. Please do not wear flip-flops or sandals if you are coming to therapy for your hip, knee, ankle, or foot.
Parents Must Accompany Minors on the First Visit
If the patient is under 18 years of age, a parent or guardian must accompany them on their first therapy visit as insurance documentation must be signed and also the therapist will often discuss your child's case with you. If a parent or guardian does not accompany the child or minor on the initial visit, then unfortunately we will have to reschedule the appointment.
Co-Pays and Payments are Collected on the Date of Service
Depending on the type of insurance you have, our office may require a co-pay or payment for service on the date of your visit. Feel free to contact your insurance company or our office and our staff will be glad to provide you with any information requested.
